Athletic Forms
To participate in Ropes ISD athletics, all student-athletes must complete and submit the required University Interscholastic League (UIL) and district-mandated forms before participating in any practices, games, or events. These forms ensure the safety and eligibility of all athletes and must be updated annually.
Deadlines & Important Information
- All forms must be submitted before the first practice of the season.
- Students will not be allowed to participate in any athletic activities without completed forms.

Submitting Forms
Ropes ISD uses [INSERT] to simplify the submission process. Parents and students can complete and upload required documents online.
Required Forms
1. Pre-Participation Physical Examination (PPE) & Medical History Form
- All student-athletes must complete a Pre-Participation Physical Examination conducted by a licensed medical professional.
- The Medical History Form must be completed by both the student and their parent/guardian to identify any health concerns that could impact participation.
2. Concussion Acknowledgement Form
- This form provides essential information on concussion risks, symptoms, and safety protocols.
- Both students and parents must sign the document acknowledging their understanding of concussion management.
3. Sudden Cardiac Arrest Awareness Form
- Educates athletes and parents on the warning signs of sudden cardiac arrest and how to respond to potential emergencies.
4. Parent/Student Steroid Agreement Form
- Required by UIL, this form ensures that students and parents understand the dangers of steroid use and agree to comply with regulations.
