Required Documents for Student Registration

Please bring the following documents to registration:

• State-issued birth certificate (hospital copies are not accepted)
• Social Security card
• Parent/guardian photo ID
• Current immunization records
• In-district students: Proof of residency (current utility bill and property deed or lease agreement)
• Athletic/Physical Form (Grades 7–12)
• Official high school transcript (Grades 9–12)
